New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address. 

As a guide to the catchment area we are bounded in the south by the seafront as far as Chalkwell Avenue. In the east, north of the railway line our boundary is Crowstone Road/Tintern avenue. To the west we go as far as Woodfield Road in Hadleigh and to the north to the Arterial Road stretching from Westbourne Grove to the Borough Boundary.

To register you will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. To process your registration you need to attend the surgery in person.  Medical treatment is available from the date of registration completion. Please contact reception for further information.

Out of Area Patient Registrations.

NHS England has released the new GP Choice specification and guidance.  From 5 January 2015 all GP practices are able to register new patients who live outside the practice area, without any obligation to provide home visits or services out of hours when the patient is unable to attend their registered practice.  Due to concerns over patient safety the Pall Mall Surgery has decided that they will not accept patient registrations for those patients living outside of our practice boundary.
